Dietitian - Level IV (Provisional Appointment) Nutrition Specialist

We are seeking a highly skilled and experienced Dietitian to join our team at NYC Health Hospitals. As a Dietitian, you will be responsible for providing medical nutrition therapy to adult patients in the weight management/bariatric practices. You will work under the direction of the coordinating manager and in coordination with the weight management/bariatric physician to develop and implement medical nutritional therapy protocols.
Key Responsibilities- Provide nutrition services for facility-based weight management/bariatric program
- Document care plans, consultations, recommendations, and follow-up interventions in the electronic medical record
- Assist in the development and implementation of medical nutritional therapy protocols
- Review clinical and laboratory data to formulate a weight management care plan
- Educate and counsel patients on the nutritional management of chronic diseases with an emphasis on obesity, pre-diabetes, and hypertension
- Provide extensive education on diet using motivational interviewing techniques
- Consult with other team members as necessary, including physicians and clinical psychologists
- Participate in hospital-wide health fairs, staff health challenges, and community-based outreach programs
- Create culturally sensitive educational materials and facilitate educational groups, shared medical appointments for program participants with attention to literacy levels and diverse patient population needs
- Offer nutrition classes, cooking demonstrations, and lectures as needed
- Participate in the development and implementation of quality improvement activities as assigned
- Reports problems to immediate supervisor and recommends ways to improve nutritional services
- Baccalaureate degree from an accredited college or university, with a major in Dietetics or Foods and Nutrition in a program approved by the American Dietetic Association (ADA); or
- Registration or eligibility for registration by the Commission on Dietetic Registration (CDR) as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist
Under the Medicare and Medicaid Programs, Reform of Requirements for Long-Term Care Facilities, as outlined in Section of Title 42 of the Code of Federal Regulations all newly-hired Dietitians, Levels I-IV assigned to a Post-Acute Care (Long-Term Care) facility, are required to have New York State Certification, issued by the New York State Education Department (NYSED), as a Dietitian-Nutritionist at the time of hire.
Dietitians, Levels I-IV hired prior to November 28, 2016, assigned to a Post-Acute Care (Long-Term Care) facility are required to obtain New York State Certification, issued by the NYSED, as a Dietitian-Nutritionist on or before November 28, 2021.
