Job Summary

We are seeking a highly motivated and compassionate Outreach Specialist to join our team at Public Health Foundation Enterprises, Inc. The successful candidate will be responsible for conducting outreach efforts in Contra Costa County to engage and stabilize homeless individuals, providing them with access to essential services and support.

Key Responsibilities
  • Conduct mobile and street outreach to engage homeless individuals and provide them with information about available services.
  • Work collaboratively with community partners, including service agencies, businesses, and community groups, to ensure clients are connected to services requested.
  • Provide transportation and support to clients as needed, and assist them with completing applications for services.
  • Build trust with and assess the needs of unsheltered homeless persons encountered during outreach efforts.
  • Distribute survival supplies, including food, hygiene supplies, blankets, and socks.
  • Administer intakes, housing assessments, and other forms documentation as needed.
  • Review and understand program data, as it pertains to reporting out with the Homeless Management Information System (HMIS).
Requirements
  • AA/AS degree in Human Services, Psychology, Counseling, or a related field.
  • OR High School degree or equivalent plus a minimum 2-year direct experience working in a professional capacity with homeless, behavioral health, or medically complex populations.
  • Desired qualifications include bilingualism, biculturalism in Spanish, lived experience of homelessness, and prior experience or training in street outreach and clinical case management.
Work Environment

The Outreach Specialist will work in a variety of environments, including general office settings, homeless shelters, and field work within Contra Costa County. The successful candidate must be able to work well with diverse staff, stigmatized communities, and clientele, and must be able to work successfully both independently and cooperatively.
