About Us:
We provide early intervention services to families with children birth to 3 years of age with developmental delays or at risk for developmental delays. Our primary focus is to provide education, training, and support to families to help their children learn and grow.
Responsibilities:
- Demonstrate knowledge and understanding of all areas of infant-toddler development.
- Utilize a parent coaching model in work with families.
- Participate in screenings, assessments, child groups, parent groups, center visits, and home visits as a member of a trans-disciplinary team.
- Provide home-based services to families across the catchment area.
- Collaborate with community providers to ensure continuity of services for families.
- Provide service coordination to assigned caseload, including coordinating the development, monitoring, and review of the Individual Family Service Plan.
Qualifications:
- Master's Degree in Social Work or related field.
- Current licensure as a Licensed Certified Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW).
- Two (2) years of pediatric experience preferred.
- Effective verbal and written communication skills.
- Working knowledge of computer-based technology, including a computer/laptop and cell phone.
- Valid driver's license and reliable transportation required.
