About the Role:

We are seeking a skilled Rheumatologist to join our integrated care team at Avita Pharmacy. As a Rheumatology Specialist, you will provide patient-centered care for individuals living with chronic rheumatologic conditions, including inflammatory and degenerative arthritides, autoimmune diseases, osteoporosis, and related disorders.

Key Responsibilities:

  • Conduct outpatient care consultations, creating diagnostic and treatment care plans.
  • Provide clinician consultation services to internal clinicians and collaborating practices.
  • Develop and implement comprehensive models of outpatient care for patients with complex chronic diseases.
  • Collaborate with clinic leaders, other specialists, and care team members to provide high-quality patient care.
  • Monitor patient conditions to minimize emergency department visits, hospital admissions, and readmissions.
  • Assist with educating patients regarding their chronic diseases and healthy choices.
  • Provide on-call coverage in rotation with other providers.
  • Supervise and collaborate with assigned advanced practice providers (APPs) as needed.

Requirements:

  • Medical degree from an accredited university.
  • Licensed Physician in the state of Georgia.
  • Board certified following specialized training.
  • American Heart Association BLS certification.
  • Experience in population health management preferred.
  • Experience in working with psychosocially and medically complex patients strongly preferred.
  • Knowledge or experience with NCQA PCMH model preferred.
  • Excellent interpersonal and communications skills.
  • Knowledge and experience using Electronic Medical Records and ability to analyze and leverage their reporting capabilities.
  • Working knowledge of Microsoft Office (Word, Outlook, PowerPoint, and Excel).
