Within our Youth and Family Services, we assist parents and young individuals striving for health and the restoration of their relationships. We collaborate with human services, child welfare systems, juvenile courts, and community-based service providers to ensure that individuals and families access the services and skills necessary for positive, healthy living.
We are currently seeking a full-time Healthcare Case Management Specialist.
Compensation starts at $40,000, contingent upon experience and education.
The role involves providing interim medical case management for all DCFS Wards within the Lead Agency's service area during the initial 45 days of custody. Responsibilities include:
- Quarterly communication with substitute caregivers to obtain updates on medical appointments, specialty visits, or emergency room visits.
- Requesting all medical records two months prior to the Annual Case Review.
- Ensuring all children in DCFS care are current with dental, vision, and hearing appointments.
- Confirming that all children in DCFS care have a primary care physician.
- Assisting substitute guardians in obtaining necessary services or resources, such as WIC.
- Conducting biannual assessments with substitute caregivers to ensure the emotional and physical stability of the children in DCFS care.
- Providing a Health Passport to caregivers as needed.
- Completing an Annual Case Review report for each child in DCFS custody.
- Maintaining a network of primary care physicians that meet Maternal Child Health Primary Care Provider standards.
- Fostering positive relationships with agencies and case managers contracted by DCFS.
- Keeping accurate records of the primary care physician selected by the substitute caregiver or ward for all eligible DCFS wards in the Lead Agency's area (SACWIS).
- Ensuring a healthcare delivery system that can provide comprehensive EPSDT program health care services to DCFS wards in the Lead Agency area.
- Developing and maintaining written procedures to inform relevant parties of the necessary actions to ensure timely access to services for DCFS wards.
- Facilitating coverage for other case managers as needed.
Qualifications:
- Bachelor's degree in social work or a related field with a minimum of two years of experience in community health or maternal and child health.
- Strong communication and organizational skills.
- Specific knowledge required for working with special populations.
- Clinical skills necessary for effective client management.
- Familiarity with the Health Insurance Portability and Accountability Act (HIPAA).
- Proficient computer skills, including Microsoft Office applications and internet navigation.
- Ability to work with diverse populations.
- Flexibility to work varied hours and travel as required (including evenings and weekends).
- Availability to attend meetings and meet client needs throughout the service area (a valid driver's license, current auto insurance, and reliable transportation are required).
