Job Summary:

The Case Management Assistant will work under the direction of the Clinical Transition Specialist (RN Case Manager) and/or Social Worker to establish appropriate discharge plans for patients. This includes making referrals, identifying transportation needs, and making arrangements, as well as completing authorization requests for medications or placement.

Key Responsibilities:

• Collaborate with RN Case Managers and Social Workers to develop discharge plans for patients
• Assist with nursing home placement to ensure a smooth transition for patients
• Maintain Medicare regulatory compliance with the delivery of Important Message documents
• Perform or assist case managers and/or social workers with daily tasks, including arranging transportation and other necessary tasks
• Schedule specialty and primary care post-discharge appointments
• Coordinate external primary and specialty services in and out of network
• Communicate post-discharge directives, appointments, and necessary information to patients and families
• Document assistance in Case Management system and Epic
• Collect data on assigned patients and ensure accurate documentation
• Provide routine office and clerical support to department staff, as appropriately delegated

Requirements:

• Three or more years of experience in a customer service-focused role, preferably in a healthcare setting, or two or more years of CNA experience
• Good problem-solving skills, self-directed, and able to complete required assigned work independently
• Effective interpersonal and communication skills with individuals in diverse professional and patient groups
• Possesses knowledge and skills to work effectively with patients and their families, including neonates, children, adolescents, adults, and the elderly
• Must have strong communication skills and work well with all employees and the public at large; observes high standards of confidentiality
• Must maintain a positive attitude when working and be able to deal with a large volume of calls and multiple tasks concurrently
