Job Summary:

The Family Support Specialist will work closely with families to identify their needs and develop a plan to address them. This will involve conducting assessments, providing information and referrals, and connecting families with community resources. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:
  • Conduct comprehensive assessments of family needs and develop individualized plans to address them
  • Provide information and referrals to families on community resources and services
  • Connect families with community resources and services, including early care and education programs, family support services, and other relevant programs
  • Work closely with families to identify and achieve their goals, including improving their overall well-being and promoting their children's development
  • Maintain accurate and up-to-date records of family interactions and services provided
  • Participate in training and professional development opportunities to enhance skills and knowledge
Requirements:
  • Bilingual, bicultural Spanish/English/Cantonese required
  • 2-3 years related experience in social service eligibility and application processing
  • 2 years experience in data entry
  • Strong oral and written communication skills
  • Professional appearance and demeanor, with professional ethics of confidentiality, ability to exercise good judgment and discretion
  • Attention to detail, ability to manage a wide variety of tasks and meet deadlines, and reliability/dependability
  • Must have knowledge of title V, Title 22 and Title XVII and the ability to learn and implement contract requirements & regulations
