Position: Foster Care Services Coordinator

Location: Remote options available

Employment Type: Full-time

Key Responsibilities:

The Foster Care Services Coordinator plays a crucial role in managing services and resources for children and families. This position also offers continuous support and training to foster caregivers, ensuring adherence to established guidelines.

  • Maintain daily updates on the Bed board in the Client Care Web system.
  • Conduct weekly updates of the Placement discharge log.
  • Monitor and report weekly on Foster Parent bed availability.
  • Secure agency respite for children currently in care.
  • Respond to referrals via the Client Care Web through email and phone communication.
  • Prepare Intake packets and gather necessary documents prior to placement.
  • Coordinate with medical clinics regarding medication lists for children.
  • Ensure collection of medical and dental documentation from providers.
  • Schedule Comprehensive Clinical Assessments as needed.
  • Develop Person Centered Plans and obtain necessary signatures.
  • Submit authorizations to the Utilization Team and keep them informed.
  • Create child profiles in the Electronic Health Record (EHR) system.
  • Manage NCTOPPS submissions for children in care.
  • Input clinical documents into the Client Care Web before placements.
  • Facilitate orientation sessions for new foster families.
  • Seek placements for children with disrupted placements.
  • Participate in meetings for transferring families to compile intake and clinical documentation.
Qualifications:
  • A Bachelor's degree in human services or a related field is required.
  • Minimum of two years of relevant experience.
  • Ability to work remotely when necessary.
  • A valid driver's license with a clean driving record is essential.
  • Successful candidates must pass a drug screening and background check.

Compensation:

Salary Range: $45,000 - $55,000
