The Clinical Pharmacist is responsible for providing high-quality, individualized pharmaceutical care and integrated pharmacy services to optimize patient medication therapy outcomes and quality of life. This role requires collaboration with the healthcare team to ensure seamless care delivery.
Key Responsibilities:
- Provide high-quality, individualized pharmaceutical care to patients
- Collaborate with the healthcare team to optimize patient medication therapy outcomes
- Develop and implement integrated pharmacy services to improve patient care
- Participate in quality improvement initiatives to enhance patient outcomes
Requirements:
- Pharmacy Doctorate or Bachelor's Degree in Pharmacy
- California Pharmacist Registered upon hire
- Board of Pharmacy Preceptor license (where applicable)
- Basic Life Support-Provider (BLS) certification (where applicable)
Preferred Qualifications:
- Acute care hospital pharmacy experience
- Completion of ASHP-accredited PGY1 residency
