Role Summary: We are seeking a compassionate and skilled Clinical Care Specialist to join our team, providing high-quality care to our clients in the comfort of their own homes.

About the Role: As a Clinical Care Specialist, you will be responsible for performing personal care activities that assist patients with activities of daily living, including personal hygiene, ambulation/transfers, dressing/shaving, and preparing meals and snacks according to instructions.

Key Responsibilities:

  • Provide patient-specific care, including providing hospice supportive care, medication reminders, and assisting with prescribed range of motion exercises.
  • Assist patients with light housekeeping activities and accompany patients on errands or medical appointments.
  • Use lifting equipment such as gait belts and Hoyer lifts to ensure patient safety.

Preferred Experience:

  • Hoyer lift operation
  • Personal care assistance
  • Range of motion exercises
  • Foley care
  • Companionship
  • Light cleaning
