Job Overview

We are seeking a compassionate Registered Nurse RN Case Manager to join our elite team of healthcare professionals. As a valued member of our team, you will have the opportunity to make a meaningful impact on the lives of our patients and their families.

About the Role

As a Registered Nurse RN Case Manager, you will be responsible for providing patient care across the entire continuum of care. You will work collaboratively with our clinical team to develop and implement personalized care plans that meet the unique needs of each patient.

Key Responsibilities

  • Provide high-quality patient care in a variety of settings, including hospice and home health environments
  • Conduct comprehensive assessments of patients' medical, social, and emotional needs
  • Develop and implement individualized care plans that address patients' unique needs and goals
  • Collaborate with healthcare providers and other stakeholders to ensure seamless coordination of care
  • Document patient progress and outcomes in accordance with regulatory requirements

Requirements

  • Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
  • Current State License as a Registered Nurse RN
  • 1 year of experience in a clinical care setting
  • Experience in hospice or home health environment is preferred
  • Nursing skills as defined by accepted nursing standards
  • Ability to handle multiple priorities, documentation requirements, and deadlines
