Job Summary

A Speech Language Pathologist opportunity has become available with HarmonyCares, a nation's largest home-based primary care practices. This role will provide speech therapy services in the provision of personal care to patients.

Job Responsibilities
As a Speech Language Pathologist, you will be responsible for providing speech therapy services including assessment, evaluation, procedures, teaching, and training activities as outlined in the patient care plan. You will document daily changes and updates to the patient care plans, including discharge planning. Additionally, you will attend staff meetings, team conferences, and educational in-services per agency requirements. You will also perform all OASIS time point assessments per Medicare criteria and submit recertification paperwork per policy and procedure.

Requirements
This role requires at least one year of experience as a home care professional Speech Language Pathologist. You must be competent in performing comprehensive assessments and OASIS-C. You should possess the ability to assess and document patient needs and formulate individualized patient care plans to meet those needs. Proficiency in Microsoft Office is also required.

Benefits
HarmonyCares offers a competitive salary range of $80,000 to $110,000 per year, depending on experience. We also offer a 401K retirement plan with company match, paid orientation and training, and home health locations in 8 states.
