Nurse Care Coordinator

We are seeking a skilled and experienced Registered Nurse (R.N.) to join our team at AlphaCare Home Health Corp. The ideal candidate will demonstrate expertise in utilizing OASIS to admit, transfer, re-certify, and discharge patients according to physician orders.
Key Responsibilities:
- Conduct initial home care visits and regularly reassess patient needs and progress
- Develop and implement care plans based on physician orders
- Complete OASIS assessments for admissions, transfers, re-certifications, resumptions of care, and discharges
- Supervise LVNs and HHAs in the delivery of patient care
- Communicate changes in patient condition to physicians and appropriate clinical staff
Requirements:
- Must be a licensed Registered Nurse (R.N.)
- Minimum of one year of nursing experience as an R.N. required
- Ability to work collaboratively and positively with patients, families, and staff
