Job Summary

The Registered Nurse (RN) is a vital member of our team, responsible for delivering high-quality patient care in the comfort of clients' homes. As a skilled nurse, you will develop, implement, and evaluate individualized patient care plans, ensuring seamless coordination with other healthcare professionals.

Key Responsibilities
  • Develop and implement comprehensive patient care plans, incorporating physician orders and patient needs.
  • Provide direct patient care, including observation, assessment, and health teaching, to ensure optimal health outcomes.
  • Coordinate care with other healthcare professionals, including physicians, therapists, and home health aides.
  • Conduct ongoing assessments and reassessments to update patient care plans as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in Agency evaluation programs and maintain ongoing education to stay current with best practices.
Requirements
  • Current Nursing licensure in your state of employment.
  • Graduate of an approved School of Nursing.
  • Minimum of six (6) months recent work experience as a home care nurse or equivalent experience.
  • Satisfactory completion of Agency RN Qualifying Exam.
  • Current CPR certification.
