We are seeking a highly skilled Clinical Pharmacist to join our team at David Grant Medical Center, Travis AFB, CA. As a Clinical Pharmacist, you will play a critical role in providing exceptional patient care and contributing to the success of our Inpatient Pharmacy team.
Key Responsibilities- Precept pharmacy students and residents in a non-supervisory role, when present.
- Ensure work areas are clean and safe according to applicable regulations.
- Manage patient-specific pharmacokinetic regimens, including vancomycin, anticoagulants, and aminoglycosides.
- Assess metabolic needs and order therapeutic parenteral nutrition products.
- Provide medication and healthcare-related in-service training to clinical staff.
- Consult with patient care teams through multi-disciplinary rounds and antimicrobial stewardship monitoring.
- Document pharmacy interventions and significant adverse medication events.
- Bachelor's degree in pharmacy or Doctor of Pharmacy (Pharm.D.)
- One year of hospital-based pharmacy experience after graduation in unit-dose distribution and IV additives and/or hospital residency required.
- Active, unrestricted Pharmacist license
- Board Certification: None
- Life Support Certifications: BLS
- Security: Must be able to pass a Government background check and obtain a Government security clearance
