for an on-call position
focused on providing urgent home health visits
when necessary.
This role serves our adult and elderly clients on a per-visit basis across various regions.
On-call hours are from 5pm to 8:30am.

Responsibilities:
+ Conducting home visits to clients within designated areas.
+ Executing assigned tasks, including medication administration, wound care, treatments, and procedures.
+ Monitoring clients' health status and reporting any changes to the Clinical Manager and Client Services Manager.
+ Following through with, executing, and accurately documenting physicians' orders.
+ Performing client assessments as required.
+ Managing cases and coordinating care.
+ Documenting observations, interventions, and evaluations related to client care management and services provided, utilizing advanced electronic medical records on a state-of-the-art tablet.
Qualifications for a Licensed Practical Nurse (LPN):
+ A valid license as a Licensed Practical Nurse in the state of Pennsylvania.
+ At least one year of recent, verifiable experience as an LPN.
+ Graduation from an accredited nursing program, as verified by school transcripts or diploma.
+ Prior experience in home care is preferred but not mandatory.
+ Ability to work independently and manage time efficiently.
+ Strong interpersonal skills.
+ Proficient computer skills; prior experience with electronic medical records (EMR) is preferred.
+ Willingness to travel to assigned cases.
