Job Summary:

Aveanna Healthcare is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in Trevose, PA. Responsibilities:

  • Conduct thorough patient assessments and develop individualized care plans under the supervision of a Registered Nurse (RN).
  • Administer medications, treatments, and therapies as prescribed by the physician, ensuring accurate and timely delivery.
  • Monitor patient progress and report any changes in condition to the supervising RN or physician, providing timely updates and recommendations.
  • Provide patient and family education on healthcare management, medication administration, and self-care techniques, promoting independence and empowerment.
  • Collaborate with the interdisciplinary team to ensure coordinated and comprehensive care delivery, fostering a culture of teamwork and open communication.
  • Maintain accurate and up-to-date patient documentation in accordance with regulatory standards and organizational policies, ensuring compliance and quality.
  • Provide emotional support and advocacy for patients and their families throughout the care continuum, demonstrating empathy and compassion.

Qualifications:

  • Current and unrestricted LPN license in the state of Pennsylvania.
  • Minimum of six months of clinical experience, with a strong foundation in nursing principles and practices.
  • Excellent clinical assessment and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and collaboratively within a multidisciplinary team, with a focus on patient-centered care and outcomes.
  • Reliable transportation and willingness to travel to patient homes within the Doylestown area, with flexibility to adapt to changing schedules and priorities.
