Adult Care Coordinator

Creative Innervisions, LLC is an organization that provides services and supports to people with developmental disabilities within Maricopa County. We are currently looking to onboard individuals to permanently fill vacancies within our Adult Day Program located in North Phoenix.
This role offers a unique opportunity to make a meaningful difference in the lives of our clients. As a member of our team, you will be responsible for providing essential support and care, including administering medications, completing daily documentation, and assisting members with activities of daily living skills.
The ideal candidate will possess a high school diploma or equivalent, be at least 18 years old, and have the ability to pass State and agency-provided training. Additionally, they must obtain and maintain a Level 1 Fingerprint Clearance Card.

Key Requirements:
- Must be at least 18 years of age
- High school diploma or equivalent

- For positions that require driving, must be able to provide a 5-year Motor Vehicle Record that is in good standing
