Company Overview:

LiveWell Choice, a division of Longevity Health, is expanding and seeking to recruit an Advanced Practice Provider - Nurse Practitioner or Physician Assistant - Floater for the state of Ohio.

Position Summary: The Advanced Practice Provider (APP) - Market Floater is a licensed and board-certified nurse practitioner or physician's assistant dedicated to serving Longevity Health members. This role involves flexibility as the APP will not be assigned to a single facility but will float across various locations within the state to address staffing needs and cover absences. The travel schedule will be determined collaboratively between the market's management and the new hire.

Core Responsibilities:
  • Deliver collaborative primary care services to a designated panel of Longevity Health members, ensuring treatment aligns with their individual care goals.
  • Implement the Longevity Health Model of Care, focusing on advanced care planning and providing care in the most suitable environment.
  • Oversee Health Plan clinical operations, including prior authorizations, medication reviews, and quality improvement initiatives.
  • Act as a trusted resource for members, their families, and nursing home staff, facilitating education on care needs and changes in health status.
  • Maintain effective communication with members' primary care providers and care teams regarding treatment plans and updates.
  • Support market objectives while managing caseloads and collaborating with team members.
  • Promote the values and services of Longevity Health among members and stakeholders.
  • Function autonomously while being accountable for clinical practice.
  • Build meaningful relationships within the nursing home setting, ensuring high-quality care for members.
  • Provide assistance with additional responsibilities as required.
Qualifications:
  • Unrestricted state licensure as an Advanced Practice Nurse or Physician's Assistant with national board certification.
  • Possession of a DEA License and active national provider number (NPI).
  • Eligibility to participate in the Medicare program as a provider.
  • Valid driver's license and proof of automobile insurance are mandatory.
  • Master's degree in Nursing or a related field from an accredited institution.
Experience:
  • A minimum of six months of experience in a clinical setting, preferably with geriatric or hospice care.
  • Proficiency in technology and computer systems is essential.
  • Strong interpersonal skills to engage effectively with patients, families, and healthcare professionals.
  • Demonstrated ability to work independently while being a team player.
  • Excellent clinical assessment and decision-making skills, particularly for chronic illness management.
