We are seeking a skilled Healthcare Assistant Specialist to join our team at Apace, formerly Region V Services. As a key member of our healthcare support staff, you will play a vital role in ensuring the well-being of individuals we serve.

Job Summary

This position is responsible for providing assistance with scheduling and attending medical appointments, maintaining accurate records, and communicating effectively with healthcare providers and family members.

Schedule

This full-time position is based on a 40-hour workweek, Monday through Friday.

Apace offers a competitive salary of $19.23-$19.82 per hour, as well as a comprehensive benefits package that includes health, dental, vision, life insurance, paid time off, and more.

Responsibilities

  • Facilitate good communication with individuals, families, and healthcare providers.
  • Provide transportation and attend to medical needs by identifying requirements, making referrals, and scheduling appointments.
  • Ensure the safe operation of vehicles and the safety of individuals supported.
  • Accompany individuals as needed to work cooperatively with practitioners and request comprehensive treatment.
  • Maintain unit files to provide a comprehensive picture of individual progress and needs.
  • Provide training and assistance in daily living needs as directed by the individual's program planning team.
  • Serve as a member of the Individual Support Planning Team and/or Behavioral Intervention Team.
  • Attend staff meetings, in-service programs, and training conferences to maintain awareness of policies and procedures.
  • Perform duties after hours or on weekends as needed to attend to individual emergencies.

Requirements

  • High school diploma, GED, or equivalent knowledge.
  • Must be at least 21 years old.
  • One year of experience in human services preferred.
  • Valid CNA certification preferred.
