Key Responsibilities:
  1. Deliver personalized patient care, addressing physical, emotional, and social needs
  2. Collaborate with multidisciplinary teams to develop and implement comprehensive treatment plans
  3. Conduct thorough assessments, identifying patients' strengths and areas for improvement
  4. Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
  5. Provide education and support on health maintenance, disease prevention, and medication management
  6. Facilitate referrals to specialists and other healthcare services as needed
  7. Maintain accurate and up-to-date patient records, adhering to regulatory requirements
  8. Offer emotional support and guidance to patients and their loved ones during challenging times
Requirements:
  • Minimum 2 years of experience in nursing or a related field, preferably in a skilled nursing facility or similar setting
  • Proficiency in medical software, equipment, and best practices
  • Superb communication and interpersonal skills, with the ability to build strong relationships with patients and colleagues
  • Capacity to work effectively in a dynamic environment, prioritizing tasks and managing time efficiently
