Job SummaryWe are seeking a highly skilled and compassionate Registered Nurse to join our Ambulatory Care team. As a key member of our primary care clinical team, you will provide exceptional patient-centered care, collaborate with providers and support staff, and contribute to the delivery of optimal healthcare outcomes.
Responsibilities- Provide clinical nursing interventions and support efficient and timely communication between primary care providers and specialists.
- Collaborate with patients, their family/designated care partner(s), and healthcare teams to develop and implement individualized care plans.
- Utilize evidence-based ambulatory care nursing interventions during clinical visits, telephone encounters, and electronic communication.
- Document all patient and family/designated care partner(s) interactions and clinical care in electronic medical records (EMRs) and nursing care given according to organizational standards and in compliance with applicable regulations.
- Perform care coordination for patients, including abnormal results, triage, RN verification of vaccinations and other injections, and management of in-baskets.
- Oversee clinical care essential to the assessment, promotion, maintenance, and restoration of patients' health and wellness.
- Minimize risk of harm to patients, their family/designated care partner(s), by following established nursing practices and protocols.
- Use information technology to communicate, manage knowledge, and mitigate errors.
- Use data to monitor the outcomes of care processes and support ethical decision-making; use improvement methods to design and test changes to continuously improve the quality and safety of patient care.
- Graduate of an accredited school of nursing required; BSN or completion of BSN within 3 years of hire strongly preferred.
- 2+ years' experience as a Nurse, in primary care or ambulatory setting preferred.
- Current RN licensure in state of employment; AHA BLS certification.
