Job Summary

Evergreen Life Services Texas is seeking a dedicated and compassionate Caregiver II - Community Support Specialist to join our team. As a Caregiver II, you will play a vital role in maintaining a healthy, safe, and therapeutic environment for individuals with intellectual and developmental disabilities.

Key Responsibilities
  • Ensure the health and well-being of individuals served, including administering medication as allowed by state guidelines.
  • Assist individuals with daily life activities, such as meal preparation, laundry, and personal hygiene.
  • Develop and implement individualized plans of care, including Active Treatment Plans and Nutritional, Behavior, and Physical Support plans.
  • Transport individuals to appointments, activities, and outings as requested.
  • Complete all required documentation, including daily notes, incident reports, and medical information.
Requirements
  • Valid driver's license with a good driving record.
  • Ability to read, write, and communicate verbally in English.
  • Meet and maintain all requirements of the applicable state agency, including good standing with the state abuse and neglect registry.
  • Successful completion of state-required medication administration certification and up-to-date training.
  • At least six months of prior experience working with individuals with intellectual and developmental disabilities.
Working Environment

As a Caregiver II, you will work in a variety of settings, both indoors and outdoors, and may be required to work evenings as needed.
