About the Job

We are seeking a skilled LPN to join our team in Doylestown, PA. As an LPN with IntelyCare, you will have the opportunity to work in post-acute facilities, providing high-quality care to residents while enjoying a flexible schedule that suits your needs.

Responsibilities
  • Provide direct patient care, including administering medications, monitoring vital signs, and documenting health concerns
  • Collaborate with healthcare teams to develop and implement resident care plans
  • Conduct intakes, document health concerns, and monitor medical history of residents
  • Administer and verify medications, monitoring their effects and reporting any concerns
  • Review residents' charts and medical records, ensuring accurate and up-to-date information
  • Assist with the cultivation of a safe and comfortable facility environment
  • Maintain appropriate stocks of medical supplies and participate in IV set up, immunizations, and phlebotomy procedures
Qualifications and Skills
  • Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of approved Licensed Practical Nurse (LPN) educational program
  • High school diploma (or GED/equivalent)
  • Minimum of 6 months of experience (LPN or related role)
  • Ability to lift a minimum of 50 lbs
