Aveanna Healthcare Private Duty Nurse LVN/LPN

Aveanna Healthcare is seeking a skilled Private Duty Nurse LVN/LPN to join our team in providing high-quality care to our patients. As a Private Duty Nurse, you will be responsible for delivering one-on-one care to our patients in their homes, assisting them with transitioning from the hospital and providing the necessary care to remain at home with their loved ones.
Key Responsibilities- Implement and manage physicians' prescribed plans of care for our patients.
- Conduct patient assessments and coordinate care to ensure the best possible outcomes.
- Administer prescribed medication, treatments, and therapies as needed.
- Document care provided in a timely and compliant manner.
- Provide health education and training to family members and caregivers.
- Accept and write verbal orders from physicians as required, and obtain co-signature from Case Manager.
- Follow up with physicians, clients, family members, staff, and Case Manager on client problems, concerns, and changes in client condition.
- Active, unencumbered license (RN/LPN/LVN) in the state of practice.
- Current CPR certification from American Red Cross, American Heart Association, or BLS.
- TB skin test (current within last 12 months).
- Six months prior hands-on nursing experience preferred but not required.
- Reliable transportation.
