Job SummaryWe are seeking a compassionate and skilled Private Duty Nurse to join our team. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes. You will work closely with our patients, their families, and our healthcare team to develop and implement personalized care plans that meet their unique needs.
Responsibilities- Implement physicians' prescribed plans of care for patients
- Conduct patient assessments and coordinate care with our healthcare team
- Administer prescribed medication, treatments, and therapies as needed
- Document care provided in our system of record in a timely and compliant manner
- Provide health education and training to patients and their families
- Accept and write verbal orders from physicians as required, and obtain co-signature of Case Manager on all verbal orders
- Follow up with patients, families, and our healthcare team on patient problems, concerns, and changes in patient condition
- Active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice
- Current CPR certification from American Red Cross, American Heart Association, or BLS
- TB skin test (current within last 12 months)
- Reliable transportation
