About the Role

We are seeking a skilled and compassionate Private Duty Nurse - LVN to join our team. As a Private Duty Nurse, you will play a vital role in providing high-quality, patient-centered care to our clients in the comfort of their own homes.

Responsibilities

  • Implement and manage patient care plans in collaboration with our interdisciplinary team
  • Conduct thorough patient assessments and coordinate care with other healthcare professionals
  • Administer prescribed medications, treatments, and therapies as ordered by physicians
  • Document patient care and progress in our electronic health record system
  • Provide education and support to patients and their families on health promotion, disease prevention, and management
  • Collaborate with our Case Management team to ensure seamless care transitions and coordination

Requirements

  • Active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice
  • Current CPR certification from American Red Cross, American Heart Association, or BLS
  • TB skin test (current within last 12 months)
  • Reliable transportation
