Job Summary: We are seeking a skilled Registered Nurse (RN) to fill the role of Care Coordinator for Pain Therapy at Fairview Health Services. As a key member of our multidisciplinary team, you will play a vital role in coordinating patient care and promoting effective pain management strategies.

Key Responsibilities:
  • Coordinate Patient Care: Collaborate with healthcare professionals to develop and implement comprehensive care plans for patients with chronic pain.
  • Provide Clinical Guidance: Serve as a clinical resource for patients, families, and healthcare teams, offering education and support to promote self-care and self-reliance.
  • Communicate with Referring Providers: Maintain open communication with referring healthcare providers to ensure seamless coordination of care and comprehensive plans.
  • Develop and Implement Education Plans: Create and implement multidisciplinary education plans to promote effective pain management strategies and self-care practices.
  • Provide Phone Consultations and Triage: Offer phone consultations and triage services to patients, families, and healthcare professionals to address pain management concerns and provide support.
  • Support Clinic Procedures: Assist with clinic procedures, monitor patient progress, and provide post-procedure education and discharge support.
Requirements:
  • Education: Associate degree in nursing (ADN) or Bachelor of Science in Nursing (BSN)
  • Experience: One year of experience as a Registered Nurse (RN)
  • License/Certification: Current Minnesota RN license
Preferred Qualifications:
  • Education: Bachelor of Science in Nursing (BSN) or Bachelor of Arts in Nursing (BS/BA)
  • Experience: Three years of experience as a Registered Nurse (RN) in a pain clinic setting
  • License/Certification: Advanced Cardiovascular Life Support (ACLS) certification
