We are seeking a skilled LPN Care Coordinator to join our team at Burnsville Carefree Living, a reputable assisted living community in Burnsville, MN.
Key Responsibilities:- Administer medications as ordered by the doctor and complete treatments and cares.
- Assist residents with Activities of Daily Living (ADLs).
- Work closely with nursing staff as a team and support others in providing resident-centered care.
- Mentor new staff and RA's, demonstrating a good role model.
- Complete medication orders, re-fills, and overall medication oversight for clients in our care.
- Conduct supervisory visits, chart audits, and general nursing practices as required by our Medicare Comprehensive License and Company policies.
- Associate degree required, with licensure as a Licensed Practical Nurse in the State of Minnesota.
- Professional and mature demeanor to work effectively and respectfully with staff, managers, executives, and others with varying working styles.
- Experience serving individuals with Alzheimer's or related dementias is beneficial.
