Senior Community Care Nurse Practitioner

At UnitedHealth Group, we're revolutionizing the way healthcare is delivered. Our Optum Home & Community Care team is seeking a skilled Nurse Practitioner or Physician Assistant to join our Senior Community Care program.
Key Responsibilities:- Conduct comprehensive assessments to identify patient needs and develop personalized care plans.
- Manage patients with acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
- Collaborate with interdisciplinary care teams to ensure seamless care coordination.
- Provide advanced illness and advanced care planning conversations to patients and caregivers.
- Document patient information accurately and completely, including ICD-10 conditions.
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (NP) or accredited Physician Assistant degree program (PA).
- Active and unrestricted license in the state of practice or ability to obtain by start date.
- Active Nurse Practitioner or Physician Assistant certification through a national board.
- Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
- 1+ years of clinical experience in a long-term care setting.
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
- Understanding of Geriatrics and Chronic Illness.
- Understanding of Advanced Illness and end-of-life discussions.
