We're seeking a highly skilled NP or PA to join our interdisciplinary team and provide comprehensive care to our highest-risk health plan members in Steuben County, NY.

Key Responsibilities:
  • Conduct thorough assessments to identify patients' needs and develop personalized care plans
  • Manage patients through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions
  • Ensure accurate and complete documentation, including ICD-10 conditions
  • Collaborate with the interdisciplinary care team to provide holistic care
  • Provide patients and caregivers with counseling and education on advanced illness and end-of-life discussions
Requirements:
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (NP) or accredited Physician Assistant degree program (PA)
  • Active and unrestricted license in the state of New York or ability to obtain by start date
  • Active Nurse Practitioner or Physician Assistant certification through a national board
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions
Preferred Qualifications:
  • 1+ years of clinical experience in a long-term care setting
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end-of-life discussions
