Position Summary

The Licensed Practical Nurse (LPN) plays a vital role in ensuring the health and wellness of residents by delivering comprehensive services in a respectful and dignified manner. Reporting directly to the Health & Wellness Director, the LPN will oversee the care provided by Certified Nursing Assistants (CNAs) and Resident Assistants (RAs), ensuring that each resident's individuality is recognized and honored. This position requires a commitment to safety and quality care.

Key Responsibilities
  • Facilitate the admission process and orient new residents, ensuring a smooth transition for both residents and their families.
  • Maintain accurate health records and monitor residents' mental and physical conditions, promptly reporting any changes to the attending physician.
  • Administer prescribed treatments and document residents' responses effectively.
  • Collaborate with physicians regarding medication management and work with pharmacy services to ensure proper medication handling.
  • Uphold and protect resident rights as outlined in the Resident Bill of Rights.
  • Implement infection control protocols and ensure compliance among staff.
  • Work with dietary teams to provide appropriate nutritional and therapeutic meals for residents.
  • Investigate and document any incidents involving residents.
  • Assist in coordinating resident discharges and transfers.
  • Engage in direct resident care and support new staff through training initiatives.
  • Meet deadlines and perform duties under potentially stressful conditions.
  • Participate in daily resident programming and activities.
  • Maintain regular and reliable attendance as a fundamental aspect of this role.
Essential Skills and Physical Requirements

The following skills and physical capabilities are necessary for effective performance in this role:
  • Proficient in reading, writing, and following verbal and written instructions in English.
  • Ability to think critically and respond appropriately to complex information.
  • Capable of walking long distances regularly.
  • Ability to lift and carry up to 50 lbs.
  • Skilled in pushing wheelchairs with residents weighing up to 200 lbs.
  • Comfortable with repetitive hand motions.
  • Able to hear and engage in ordinary conversations and office sounds.
  • Effective verbal communication skills in person and over the phone.
  • Proficient in written communication, both by hand and on a computer.
Qualifications

Minimum Requirements
  • Current Florida LPN license; must maintain licensure.
  • Completion of an LPN Supervisory Course or willingness to complete within six months of hire.
  • Two or more years of relevant experience in geriatrics within long-term care, home health, or similar healthcare settings.
  • Strong verbal and written communication abilities.
  • Computer literacy, including proficiency in Microsoft Office Suite.
  • Exceptional interpersonal and communication skills.
