Role: Physical Therapist

Summary:
The Physical Therapist is responsible for delivering specialized services aimed at identifying, preventing, and rehabilitating physical dysfunction or discomfort. This role involves evaluating patients and implementing therapeutic techniques, including physical and chemical modalities, exercise regimens, and patient education. The therapist collaborates with patients, their representatives, and caregivers to create a personalized care plan, while maintaining communication with the overseeing physician and other healthcare professionals.

Key Responsibilities:

1. Conduct comprehensive physical therapy assessments, focusing on functional limitations, mobility, endurance, and activities of daily living (ADLs).
2. Identify additional home health service needs for patients and facilitate referrals as necessary.
3. Administer treatments as prescribed by the physician, ensuring effective communication with all involved healthcare providers.
4. Implement appropriate therapeutic interventions to alleviate pain, enhance function, and promote independence using suitable equipment and techniques.
5. Document all evaluation findings, treatment plans, and patient responses to therapy during each visit, contributing to discharge planning.
6. Engage in interdisciplinary case management meetings to ensure cohesive physical therapy and overall care coordination.
7. Provide education and counseling to patients, families, and healthcare team members regarding treatment protocols, documenting the recipients of this information.
8. Arrange for necessary equipment to support patient independence and functionality.
9. Participate in quality assurance initiatives and ongoing training programs sponsored by the agency.
10. Supervise the care provided by Physical Therapist Assistants (PTAs) and home health aides as appropriate.
11. Adhere to all compliance standards relevant to the position.
12. Perform additional duties as assigned.

Qualifications:

Required:
Bachelor's degree in Physical Therapy
Minimum of one (1) year of relevant experience
Current Physical Therapist licensure

Preferred:
Experience within a recognized home health agency.

Working Conditions:
Essential physical demands include significant manual dexterity, exposure to various occupational risks, and the ability to exert substantial force when necessary.
