Physical Therapist Per Visit

Western CT Home Care Inc is seeking a skilled Physical Therapist to join our team on a per diem basis. As a Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients in their homes.
Job Summary:The Physical Therapist will evaluate and analyze patients to identify their physical dysfunction or pain, and develop a personalized plan of care to remediate and rehabilitate their condition. The Physical Therapist will work closely with the patient, their representative (if any), and caregiver to ensure that the patient receives the best possible care.
Responsibilities:- Evaluate and analyze patients to identify their physical dysfunction or pain.
- Develop a personalized plan of care to remediate and rehabilitate the patient's condition.
- Provide physical therapy services to patients in their homes, including exercises, instruction, and other procedures as needed.
- Communicate with the patient's physician and other healthcare providers as necessary.
- Document patient progress and outcomes.
- Participate in case management conferences with the healthcare team to coordinate patient care.
- Provide education and counseling to patients and their families on the patient's treatment regimen.
- Supervise physical therapy assistants and home health aides as needed.
- Fulfill all compliance responsibilities related to the position.
- Bachelor's degree in Physical Therapy.
- Current Physical Therapist license.
- Minimum one year of experience in a qualified home health agency.
The Physical Therapist will work in a home health care setting, providing services to patients in their homes. The Physical Therapist will be required to travel to patient homes, and may be required to work in a variety of settings, including hospitals and rehabilitation centers.
