Job Summary:

The Physical Therapist will be responsible for evaluating and treating patients with physical dysfunction or pain. You will work closely with patients, their families, and other healthcare professionals to develop and implement individualized plans of care.

Responsibilities:
  • Evaluate patients to identify physical limitations and develop treatment plans to improve function and independence.
  • Provide physical therapy services, including exercises, instruction, and education to patients and their families.
  • Communicate with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Document patient progress and treatment plans to ensure accurate and effective care.
  • Participate in case management conferences to coordinate care and ensure optimal patient outcomes.
Requirements:
  • Bachelor's degree in Physical Therapy.
  • Current Physical Therapist license.
  • Minimum one year of experience in home health or a related field.
  • Preferably experience in a qualified home health agency.
Work Environment:

This is a per diem position, and the Physical Therapist will work in a home health setting, visiting patients in their homes. The ideal candidate will have excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team.
