Senior Case Manager II

Job Summary:
The Senior Case Manager II is a key member of our case management team, responsible for mentoring and guiding team members to ensure the delivery of high-quality services to our clients. This role requires a strong understanding of case management principles and practices, as well as excellent communication and interpersonal skills.
Key Responsibilities:
- Conduct comprehensive assessments and develop individualized service plans to meet the unique needs of each client
- Collaborate with team members to ensure seamless service delivery and effective case management
- Provide training and guidance to junior case managers to enhance their skills and knowledge
- Monitor and evaluate the effectiveness of case management services and make recommendations for improvement
- Develop and maintain relationships with community partners and service providers to ensure access to necessary resources
Requirements:
- Bachelor's degree in social work or a related field
- Minimum of 2 years of experience in case management, preferably in a non-profit or social services setting
- Strong knowledge of case management principles and practices
- Excellent communication and interpersonal skills
- Ability to work effectively in a team environment
Preferred Qualifications:
- Master's degree in social work or a related field
- Experience in supervision or lead role
- Valid driver's license
- HMIS navigation skills
