SUMMARY

Heluna Health is dedicated to addressing homelessness through its Housing for Health (HFH) initiative, a vital component of the Health Services Administration under the Los Angeles County Department of Health Services (DHS). Our organization promotes a flexible work environment, allowing staff to work both remotely and on-site as necessary.

The Medical Case Worker, part of the Client Wellness Team, operates under the guidance of the Senior Clinical Social Worker. This role focuses on evaluating the needs, circumstances, strengths, and support systems of clients within the ERC facility. The primary objective is to assist homeless individuals in transitioning to stable housing while providing holistic care in a licensed 24-hour care environment.

Additionally, the Medical Case Worker will work closely with clients to create and implement tailored care plans that address their unique needs and aspirations. Responsibilities include conducting needs assessments, delivering supportive services, managing cases, maintaining accurate documentation, advocating for clients, and performing essential functions as outlined below.

ESSENTIAL FUNCTIONS

Under the supervision of professional staff, the Medical Case Worker will be an integral member of multidisciplinary clinical and program teams, providing services to clients in licensed care facilities, including but not limited to:

  • Performing thorough assessments of clients' medical, social, and psychological requirements.
  • Conducting in-person visits to clients in facilities to complete initial housing acuity index assessments and collaboratively develop individualized care plans within the first 90 days.
  • Providing ongoing case management and coordinating services according to established care plans during the initial 90 days of residency, including but not limited to:
  • Delivering bi-weekly case management services for clients who have been in the facility for 4-6 months.
  • Utilizing environmental data related to medical or mental health issues to recommend services that maximize patient benefits.
  • Coordinating and monitoring care plan implementation to ensure timely access to necessary medical services.
  • Regularly assessing and updating care plans based on client progress and evolving needs.
  • Maintaining precise and current documentation of client assessments, care plans, interventions, and progress.
  • Collaborating with insurance providers, public agencies, and community organizations to advocate for clients' access to essential resources and services.
  • Adhering to legal and ethical standards regarding client confidentiality and information security.
  • Championing clients' rights and assisting in overcoming barriers to healthcare access.
  • Educating clients on their living conditions and self-care practices.
  • Helping clients understand and comply with prescribed medication regimens and healthcare recommendations.
  • Providing information and referrals to relevant community resources and support services.
  • Working with healthcare professionals, social workers, and other stakeholders to ensure effective communication and care coordination.
  • Facilitating communication among clients, facility operators, and healthcare teams to ensure all parties are informed and involved in decision-making.
  • Participating in interdisciplinary meetings and sharing insights regarding clients' medical and psychosocial needs.
  • Preparing reports summarizing clients' medical and social situations, highlighting significant changes or concerns.
  • Making referrals and collaborating with various public and private health, mental health, or other agencies to address clients' personal and social challenges.
JOB QUALIFICATIONS

Three years of experience in case management, preferably in a healthcare context, as a medical case worker or case manager, assisting clients with social challenges - OR - A Bachelor's degree from an accredited institution and one year of professional casework experience in interviewing, counseling, and assisting clients with social issues.

Certificates/Licenses/Clearances
  • A valid California Class C Driver License or the ability to utilize alternative transportation methods as required for job functions.
  • Successful completion of the Live Scan and Health Clearance process with the County of Los Angeles.
Other Skills, Knowledge, and Abilities
  • Experience in homeless healthcare.
  • Experience delivering clinical care services within ARFs and RCFEs.
  • Familiarity with complex care management for high-need/high-cost medical beneficiaries.
  • Ability to lead teams in performance improvement initiatives.
  • Strong teaching and communication skills with both clinical and non-clinical staff.
  • Expertise in assessing and managing clients with substance use disorders, including knowledge of Medication-Assisted Treatment (MAT) and Addiction Psychiatry.
  • Expertise in assessing and managing clients with serious persistent mental illness.
  • Experience in trauma-informed care and harm reduction strategies.
  • Familiarity with DMH, DPH, and community mental health and substance use programs and resources.
  • Excellent communication and interpersonal skills, capable of building rapport with diverse individuals.
  • Strong organizational and problem-solving skills, with empathy and a genuine desire to assist others.
  • Knowledge of relevant laws, regulations, and ethical standards in healthcare.
  • Proficiency in healthcare software and documentation systems.
  • Comprehensive understanding of medical terminology, healthcare systems, and community resources.
