Position Summary:

The Patient Care Coordination Specialist plays a vital role in ensuring that patients receive comprehensive support throughout their healthcare journey. This position involves collaborating with a dedicated team to assess patient needs, gather essential information, and utilize this data to recommend appropriate care pathways.

Key Responsibilities:

  1. Collects and processes essential information for new patient admissions, ensuring accurate digital record-keeping.
  2. Provides essential administrative support, including digital filing, copying, and faxing tasks.
  3. Facilitates referrals to other healthcare professionals as necessary.
  4. Confirms and manages patient appointments effectively.
  5. Maintains accurate records of appointment schedules.
  6. Responds to inquiries via phone and email in a professional manner.
  7. Coordinates scheduling calls and follow-ups across multiple office locations.
  8. Participates in weekly meetings to discuss patient care and recommendations with the healthcare team.
  9. Assists in resolving issues while maintaining patient confidentiality.
  10. Keeps detailed documentation of patient interactions, concerns, and feedback.

Qualifications:

  1. Experience in call center operations and customer service is essential.
  2. Proficient in appointment scheduling and record-keeping practices.
  3. Exceptional organizational skills are required.
  4. Strong verbal, written, and interpersonal communication abilities.
  5. Familiarity with the healthcare industry is beneficial.
  6. Ability to prioritize tasks and manage multiple responsibilities effectively.
  7. Meticulous attention to detail is necessary.
  8. Proficient in Microsoft Office applications, particularly Word and Excel.
  9. Experience with eClinical Works is preferred.
  10. Knowledge of personal injury cases is advantageous.
  11. Understanding of medical terminology is a plus.

Education and Experience:

  • High School Diploma or GED is required.
  • 1-3 years of relevant experience in a medical office or call center setting.

Work Environment:

This position is primarily based in a professional office setting, utilizing standard office equipment such as computers, phones, copiers, and fax machines.

Physical Requirements:

The physical demands of this role include:

  • Primarily sedentary work, with occasional filing that may require lifting files and bending.
  • Ability to lift office supplies and materials weighing up to twenty pounds.
  • Visual acuity sufficient for reading printed materials and computer screens.
  • Auditory capability to perform job duties effectively in person and over the phone.
  • Clear verbal communication skills for interactions in person and via telephone.

Position Type:

This is a full-time role, requiring a commitment of at least 40 hours per week, with variable hours based on project needs.
