We are seeking a qualified Licensed Practical Nurse (LPN) to join our team at Outreach Development Corporation. As a LPN, you will play a vital role in providing health services to our clients in a men's residential facility located in Brentwood, Long Island.
Job Summary:The LPN will be responsible for delivering and implementing patient health services, working as a member of the treatment team. This is a part-time position, working Saturday and Sunday, 7:00 AM - 5:00 PM.
Key Responsibilities:- Conduct and/or monitor all medication calls and health calls as scheduled.
- Work with clients and guardians to schedule medical appointments and conduct follow-up as required.
- Work with the Agency regarding client medication and pharmacy refills to ensure prescriptions are filled/re-filled in a timely manner.
- Provide on-site health services to address emerging and/or current health needs of clients.
- Provide training and guidance to key staff members in emergency health situations.
- Establish and monitor health-related policies and procedures to ensure a safe and healthy environment.
- Provide immediate first aid in health emergencies and monitor situations until emergency services arrive.
- NYS Licensed Practical Nurse (LPN) certification.
- Experience working in a substance abuse treatment setting or special populations required.
- Experience working with adolescents a plus.
- Bilingual a plus.
- Ability to work one of the schedule options listed above.
- Ability to work with high-risk adolescents.
- Flexibility in scheduling a plus.
- Ability to work effectively in collaboration with diverse groups of people.
