We are seeking a highly skilled Clinical Pharmacogenomics Pharmacist to join our team at Ochsner Health System. As a key member of our pharmacogenomics team, you will play a critical role in the medication management of assigned patients, ensuring that their medication therapy is appropriate and effective.

Key Responsibilities:

  • Monitor and evaluate medication therapy for appropriateness based on patient health status, institutional criteria, and cost-efficient measures.
  • Provide education to healthcare providers and patients on medication management and pharmacogenomics.
  • Develop and implement pathways, best practices, and guidelines for assigned patient populations.
  • Mentor residents, students, and new staff to meet departmental goals.
  • Collaborate with healthcare teams to ensure seamless transitions of care.

Requirements:

  • Doctor of Pharmacy (PharmD) degree or equivalent.
  • Completion of a PGY1 Pharmacy Practice Residency or 3 years of related specialty experience.
  • Current Pharmacy license in the state of practice.
  • Strong leadership and teaching skills.

Preferred Qualifications:

  • Completion of a PGY2 Specialty Pharmacy Residency.
  • Experience with pharmacogenomics and medication management.
