We are seeking a skilled Registered Nurse to join our team at Harmony Home Health and Hospice. As a key member of our care team, you will be responsible for creating and managing personalized care plans for patients in their homes.
Responsibilities- Coordinate with colleagues to assess, plan, implement, or evaluate patient care plans
- Administer medications and monitor patients for side effects or reactions
- Document any changes in patient conditions and report on changes
- Maintain accurate records of patient care and discuss observations with the supervisor
- Currently licensed Registered Nurse (RN) in state of employment
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid driver's license
- First aid/CPR certification required
