Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Hospice team at Presbyterian Healthcare Services. As an LPN, you will provide high-quality patient care in a Hospice setting, working collaboratively with RN Care Managers and Medical Providers to deliver exceptional patient outcomes.

Responsibilities
  • Provide patient care in a Hospice setting, adhering to established standards of care.
  • Collaborate with RN Care Managers and Medical Providers to develop and implement patient care plans.
  • Conduct thorough assessments of patients' physical and emotional needs.
  • Develop and implement individualized care plans to meet patients' unique needs.
  • Provide education and support to patients and their families.
  • Participate in interdisciplinary team meetings to discuss patient care and progress.
Requirements
  • High School Diploma or GED.
  • Practical Nursing Formal Education approved by the NM Board of Nursing.
  • Licensed Practical Nurse-NM.
  • BLS required.
  • PDS IV Certification course (within orientation period).
  • Valid NM Drivers license; meets qualifications of PHS drivers policy.
