Compensation PackageWe offer a competitive salary package of up to $80,000 annually, plus incentive pay for home visits. This role is perfect for experienced Homecare Nurses or New Graduates looking for a fulfilling career in home healthcare.
Job DescriptionThis position involves providing thorough initial and ongoing assessments to identify each client's unique medical needs and level of functioning. As an RN or LPN, you will initiate and adapt the plan of care, perform OASIS assessments when needed, and coordinate services seamlessly with physicians and colleagues.
You will work closely with patients, families, and other healthcare providers to ensure that each client receives personalized and holistic care. Your attention to detail and ability to prepare clinical notes and submit required paperwork punctually are crucial to your success in this role.
Required Skills and Qualifications- RN or LPN provides initial and ongoing assessments to identify the client's medical needs level of functioning and ability to communicate and learn.
- RN or LPN initiates the plan of care necessary revisions and performs OASIS assessments at appropriate time points when applicable.
- RN or LPN initiates and performs appropriate nursing interventions.
- RN or LPN coordinates service and informs the physician and other personnel of changes in the client's condition and needs.
- RN or LPN collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
- RN or LPN prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
