Nurse Practitioner

CVS Health is seeking a highly skilled and compassionate Nurse Practitioner to join our Primary Care team. As a key member of our clinical care team, you will provide high-quality, patient-centered care to a panel of patients, working collaboratively with a dedicated team to drive better outcomes.
Key Responsibilities- Provide accurate and comprehensive assessments, diagnoses, and treatment plans for patients, utilizing evidence-based care and best practices.
- Collaborate with the clinical care team to ensure seamless care coordination and communication, promoting a culture of mutual respect and trust.
- Develop and implement effective health education and counseling programs to empower patients and families to manage their health and wellness.
- Participate in quality improvement initiatives, contributing to the development and implementation of strategies to enhance patient care and outcomes.
- Work effectively in a fast-paced, dynamic environment, prioritizing tasks and managing multiple responsibilities with ease.
- Master's degree in Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice role.
- Minimum of 1 year of recent Primary Care experience as a Nurse Practitioner in either a family medicine or primary care practice, or successful completion of CVS Minute Clinic primary care training program.
- Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
- Excellent communication, interpersonal, and problem-solving skills, with the ability to work effectively in a team environment.
