Key Responsibilities:- Provide accurate and comprehensive assessments, diagnoses, and treatment plans for patients
- Collaborate with the clinical care team to ensure seamless care coordination and communication
- Develop and implement patient-centered care plans, incorporating evidence-based practices and guidelines
- Conduct health education and counseling sessions to promote healthy behaviors and disease prevention
- Document patient care and outcomes in a timely and accurate manner
- Master's degree in Family Nurse Practitioner or Physician Assistant Studies
- Current National Board Certification and State of Employment license to practice in the Advanced Practice role
- Minimum 1 year of recent Primary Care experience as a NP or PA, or successful completion of CVS Minute Clinic primary care training program
- Working knowledge of quality measurement in the management of chronic disease conditions
- Excellent communication, organizational, and problem-solving skills
