Job Summary

CVS Health is seeking a highly skilled and compassionate Nurse Practitioner to join our Primary Care team. As a key member of our clinical care team, you will provide evidence-based care to a panel of patients, working collaboratively with a dedicated team to drive better health outcomes.

Key Responsibilities
  • Patient-Centered Quality and Safety
    • Utilize your expertise in medical conditions, risk factors, and pathophysiology to provide high-quality care to patients.
    • Conduct thorough and accurate patient assessments, including history and physical examinations.
    • Provide patient education and counseling on health maintenance, disease prevention, and treatment options.
    • Evaluate and treat patients' medical needs with safe and effective treatment plans.
    • Document all patient care within our electronic health record system, adhering to current policies and procedures.
    • Collaborate with the clinical care team to ensure seamless care coordination and patient satisfaction.
  • Communication and Interdisciplinary Collaboration
    • Work effectively with the clinical care team to deliver high-quality, patient-centered care.
    • Participate in clinical care team huddles and contribute to the development of care plans.
    • Communicate effectively with patients, families, and healthcare professionals to ensure optimal care outcomes.
    • On-call rotation with the clinical care team to provide after-hours patient care and support.
  • Healthcare Environment Management
    • Resolve conflicts and issues in a professional and respectful manner.
    • Maintain positive relationships with clinical care team members and promote a culture of respect and trust.
    • Manage clinical and non-clinical duties efficiently, prioritizing tasks and adapting to changing circumstances.
    • Enhance operational effectiveness, focusing on cost containment and quality improvement.

    • Contribute to the success of your practice by implementing business strategies aligned with our Primary Care business plan.

    Requirements
    • Master's degree in Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice role.
    • Active Advance Practice Registered Nurse or Nurse Practitioner license in good standing.
    • Current National Board Certification in Family Practice.
    • A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner in either a family medicine or primary care practice, or successful completion of CVS Minute Clinic primary care training program.
    • Working knowledge and understanding of quality measurement in the management of chronic disease conditions.
    • Self-motivated, prioritizes and solves problems, takes initiative, and advocates for patients and practice.
    • Effective verbal, written, and electronic communication skills.
    • Outstanding organizational skills and ability to multi-task.
    • Initiative, problem-solving ability, adaptability, and flexibility.
    • Ability to work remotely in a clinical care team culture.
    • Ability to work without direct supervision and practice autonomously.
    • Proficient with information management and technology.
    • Willingness to obtain multi-state licensures.
    • Capacity to collaborate with professional colleagues frequently to ensure quality patient care.
