Job SummaryWe are seeking an experienced Nurse Practitioner to join our team of healthcare professionals. As a key member of our geriatric care team, you will be responsible for delivering primary, urgent, and acute medical care to patients in their homes. Your expertise in end-of-life, geriatrics, internal medicine, and palliative care will be invaluable in providing comprehensive care to our patients.
Responsibilities- Provide primary care to patients in their homes, including routine check-ups, urgent care, and acute medical interventions.
- Collaborate with our interdisciplinary team to develop and implement individualized care plans for patients.
- Conduct initial patient screenings, physical examinations, and create assessment plans.
- Order, interpret, and analyze diagnostic tests, and determine the need for follow-up appointments and further treatment options.
- Prescribe and manage medications and treatments as appropriate, including psychopharmacological agents.
- Update patient records and ensure accuracy at each patient appointment.
- Deliver quality care while maintaining company metrics on documentation, coding, HEDIS measures, patient engagement, and medical expense.
- Participate in regularly scheduled team meetings and maintain required licensure and certification.
- Master's Degree in Nursing.
- Valid nurse practitioner or physician assistant license in the state of operations and current board certification in your field.
- 5+ years of progressive care experience.
- BLS and ACLS certified and an NPI# as an NP.
- Valid driver's license, car insurance, access to a car, and willingness to drive to patient homes/patient locations.
