Job Summary

This position is located in our Transitional Care department, which supports our cardiac program, specifically heart failure. The Advanced Practice Provider (APP) will focus on inpatient rounding for all cardiac patients, excluding those undergoing cardiac surgery. The APP will work closely with a rounding cardiologist to provide comprehensive care.

Key Responsibilities
  • Conduct new patient consults and discharges
  • Promote continuity of care and arrange follow-up visits
  • Work in the Transitional Care clinic for initial follow-up visits and coordinate with cardiologists for subsequent visits
  • Collaborate with APPs in Transitional Care to ensure cardiac patients receive necessary resources during hospitalization and post-discharge
