Job Summary: We are seeking a compassionate Nurse Practitioner to join our team, providing evidence-based medical care to seniors and patients with complex needs. As a key member of our healthcare team, you will perform examinations, treat chronic and urgent conditions, and maintain up-to-date medical records.

Responsibilities:

  • Perform comprehensive medical evaluations and create personalized treatment plans
  • Collaborate with healthcare professionals to ensure interdisciplinary care and maintain accurate patient records
  • Provide referrals to other healthcare practitioners when necessary and order laboratory and diagnostic testing as needed
  • Maintain current knowledge of medical instruments and equipment, HIPAA and OSHA regulations, and electronic medical record systems
  • Engage in moderate physical activity, including handling objects up to 25 lbs., standing or walking for more than four hours per day

Requirements:

  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
  • Strong interpersonal communication skills – verbal and written
  • Excellent time management skills with accountability for practitioner's daily workflow
