We are seeking a compassionate and skilled Nurse Practitioner to join our team at Abode Care Partners. As a House Calls Nurse Practitioner, you will play a critical role in providing personalized medical care to seniors and patients with complex needs in their homes or living facilities.

About the Role:

  • Plan, direct, and evaluate patient medical needs through evidence-based practices.
  • Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
  • Order and interpret laboratory and diagnostic testing as needed.
  • Collaborate with caregivers and other healthcare professionals for comprehensive care and maintain accurate patient records.

Requirements:

  • Active state Nurse Practitioners license.
  • Current Nurse Practitioner National Board Certification.
  • Active state DEA licensure.
  • Master's degree in Nursing (MSN).
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
